The work resulted in an abstract that was presented as a poster at the ORS in 2008. The title was: “Muscle Function is Critical for Successful Fracture Healing”. His co-authors were Gross, Asuk, Becker, Nork, Bain. This manuscript was also presented locally for Dr. Ruhlman’s resident research project.

This research has not resulted in publication directly as yet. However, it is a critical component of the upcoming
manuscript(s) on muscle function and fracture healing. The manuscript in preparation is “Transient muscle paralysis
impairs the osteogenic response to injury”.
Additional Grants and Awards
This research has continued to be an important part of the research lab at the University of Washington. The initial work by Dr. Ruhlman looking at transient muscle paralysis and fracture healing has resulted in ongoing studies funded by the Ted Hansen Chair, the Foundation for Orthopaedic Trauma, and Industry (Zimmer, Synthes).

Project Status
Project Ongoing. The project will continue to acquire data and hopefully result in future
publications. Unfortunately, patient recruitment for the study has been extremely difficult to this point. This issue has attempted to be addressed by sending out multiple mailings to potential subjects, as well as following up on these mailings with phone calls. The database from which potential subjects are drawn was also expanded by looking further back in hospital records using billing data to identify potential subjects who sustained their injuries prior to the implementation of the currently used prospective patient tracking database. In addition, an addendum to the study protocol is in the process of being evaluated by our IRB to make participation in the study more convenient for patients in order to boost recruitment. A collaboration has also been formed with a trauma center in Argentina to help increase available patient data. It is hoped that with the addition of more data, significant conclusions will be reached resulting in future publications.

1999 – Jeremy Feitelson, a graduate student in my lab, conducted his Master’s thesis research in one area of this grant. He has now enrolled in the doctoral program and plans to conduct his dissertation research in a bone blood flow area. He has not decided if his PhD research will continue in the same area or a new area of bone blood flow regulation. Additional papers and presentations related to the topic of the grant will be forth coming if he decides to continue in the thesis research area for his dissertation research. However, since the OTA funding expired some time ago, funding for this future research must come from other sources.

Project Status and Long-Term Impact of Research Award: This particular study has won significant awards wherever it has gone. It has been presented to the Canadian Orthopaedic Association, Foot & Ankle portion of the American Academy, Orthopaedic Trauma Association and to the American Academy Specialty Day. It won the Samson Award ($15,000) for best research submitted to the Canadian Orthopaedic Association Meeting. It won the Bovill Award as the best research for the year 2000 at the OTA. It has continued to enlighten people about calcaneal fractures and will be the definitive paper on displaced intra-articular fracture management when it is published. Initial funding was instrumental in its reaching these heights.

The orthopaedic axiom that all open fractures should remain open until redebrided and closed in a delayed fashion dates back to Trueta. It has never been tested in modern times by a randomized prospective trial. It is currently the practice of many surgeons to primarily close open fractures, if closeable, after aggressive debridement and irrigation. The argument supporting this practice is that the wound will never be cleaner than it is at that period of time, and with a closed wound, the wound is protected from possible contamination by resistant organisms from the hospital. Literature on this topic is based on anecdotal experiences, personal exclamations, and “no true science” and does not currently support this approach in adult patients. This project has been designed to determine if there is a clinical and economic advantage to the patient with immediate fracture closure. Thirty North American trauma centers are participating in the study and beginning August 2001 data-entry from the twenty-three sites has been web-based.
